Payment and Insurance

I accept money transfer app payments (Venmo is preferred) and all major credit cards. If you choose to pay by credit card, I will send you a credit card authorization form to keep in your file. If we have not made previous payment arrangements, you can assume that your card on file will be charged on the same day of your scheduled session.

Unfortunately, I do not accept insurance. However, if you have an insurance plan that allows for out of network coverage (typically a PPO/EPO), I am happy to provide you with a superbill to be used for reimbursement. Superbills are created on a quarterly basis only (distributed end of the month on March , June, September, and December).

Please Note: Reimbursement by your insurance company is not guaranteed and is the responsibility of the patient. Unfortunately, I am unable to negotiate the decision made by your insurance company as to whether or not you are reimbursed given I am not under contract with any insurance panel(s).

Cancellation Policy

I require written notice (text/email is preferred) for all cancellations: 732.256.1559 or thomas@thomasblaketherapy.com

Please Note: Anything within 24 hours will be charged the full session fee for the missed appointment as I cannot fill your held space without sufficient notice. For late cancellations where I am able to reschedule you within the same week of the missed session, you will only be charged for the attended session.

Contact and Emergencies

Please only use phone, email, and text for minimal use contact as these methods are not to be used as a time to process content or in lieu of your therapy session. Any contact/time spent outside of session that exceeds 15 minutes will incur our agreed upon session fee (please see your signed informed consent agreement for further detail).

All non-emergency contact attempts will be responded to by the end of the business day, unless made outside of my standard business hours (Monday through Friday: 9am to 6pm). Contact made outside of my standard business hours will be returned on the next business day.
